SWARM uses to contact parents/guardians as well as emergency information. Contents are shared among team leads only on a need-to-know basis.
SWARM Code of Conduct
SWARM expects certain behavior from its team members. Team leadership addresses exemplary and inappropriate actions.
SWARM Website Permission Form
SWARM uses the Internet for community outreach, education, and promoting our team and FIRST robotics. Using images of team members is a big part of that.
FIRST Consent and Release
Log in to www.firstinspires.org and click on the “Complete Youth Team Member Registration” button on the Dashboard to navigate to the Youth Team Member Registration. If you are a new user, you may create an account by clicking the “Sign Up” link on the top-right corner of our website.
This Greater Albany Public Schools form allows students to ride in cars driven by other team members (typically mentors)
FIRST Mentor registration
FIRST requires mentors be invited by the lead mentor. Please see the lead mentor for details.
ODE Volunteer Application and Criminal Background Check
We use the forms available on the WAHS site’s Parent and Student forms page. Please choose WAHS and use the second school of SAHS.
Driving Passengers Form
Optional: For those who drive other team members. The district requires a scan of your insurance card to keep on file.
